What it is
Violence is a major driver of ill health, trauma and long-term public service demand.
In 2015, in partnership with clinicians at the Royal London Hospital, we established the UK’s first hospital-based violence reduction service. Based on successful US models, the work intervenes at a critical “reachable moment” when young people are admitted with violence-related injuries and are most open to change.
Our professionally trained practitioners - many with lived experience of violence and gangs - work alongside clinical teams in A&E and Major Trauma Centres to provide intensive, one-to-one support in hospital and after discharge. They address the underlying drivers of harm, including trauma, housing instability, exploitation, education disengagement and mental health needs, coordinating support across local systems.
The impact is clear. A five-year independent study at the Royal London Hospital showed re-admission rates of just 7%, compared with a national average of 30–35% for this cohort. This represents significant avoided costs to health, policing and criminal justice systems, alongside improved safety and life outcomes for young people.
The model is now embedded across three regions and supports young people affected by weapons-related harm, sexual violence and acute mental health crises. Early intervention in A&E prevents escalation, while intensive trauma-centred support in Major Trauma settings supports those with the highest levels of need.
This work demonstrates how targeted, relational interventions delivered at the right moment can reduce repeat harm, improve outcomes and deliver strong public value. Continued investment is essential to sustain and scale this proven approach in areas experiencing violence and pressure on frontline services.
